Day 30: Customer Information Printable

Today's printable was a request from Cynthia who is an Origami Owl Designer. She wanted an customer information printable in a half size. This would also work great in a full size notebook with two customers to a page. Cynthia wanted to keep track of these things: customer name, address, phone, email, birthday, anniversary, family, notes, party hosted. She also needs a list for items purchased with two small check boxes on the end for personal or gift. She likes to do something special for customers, and this will help her keep track of important dates for them and what they have already purchased. She sometimes does a birthstone for their children or a charm for their anniversary.
